IT Band Syndrome of the Hip

Iliotibial band syndrome is inflammation of the iliotibial (IT) band, a stretch of fibrous tissue that runs from the hip to below the knee on the outer side of the leg. It develops from repetitive irritation and frequently affects runners during intensive training.

The condition occurs where the thighbone meets a bursa. When the band becomes inflamed, it cannot glide smoothly, causing pain and discomfort. Endurance athletes and those who rapidly increase activity levels are especially susceptible.

Symptoms of Iliotibial Band Syndrome

  • Pain over the outside of the knee joint
  • Swelling at the location of discomfort
  • A snapping or popping sensation as the knee is bent

Who is at risk?

IT band syndrome most commonly affects:

  • Distance runners and cyclists
  • Athletes who rapidly increase training intensity or mileage
  • Endurance athletes who train without adequate recovery time

Treatment for Iliotibial Band Syndrome

Treatment is designed to alleviate inflammation and rarely includes surgery. Recommended approaches include:
  • Decreasing the activity causing the condition
  • Icing the affected area
  • NSAID pain relievers such as ibuprofen and naproxen
  • Cortisone shots if other treatments fail
After inflammation subsides, physical therapy programs emphasizing stretching and strengthening the hip and knee may be recommended. Cross-training helps maintain endurance while managing discomfort. If conservative approaches fail, minimally invasive surgery to release the tendon and decrease friction may be performed.
